Implants

The implants have been around a few tens of years, but lately they occupy a leading position as an alternative treatment in the different cases of tooth loss.

The implant is introduced into the bone of the jaw of an artificial root made of titanium that will support a prosthetic tooth or an element.

Implementation of the implant

Expected2 months after the extractions before starting the first phase.

It performs a radio (scanner) required to evaluate the quality and quantity of remaining bone for implantation.
It operates a radio in 3 dimension that will give us the width height and thickness of the bone and allow us to choose the appropriate implant.

The first step is to open the gum and place the implant in bone. The gum is then closed again.

A waiting period allows the osteointegration and varies from 3 a 6 months (a period that allows the implant to settle heavily in the production of bone by bone cells that will surround the implant and the bone was fixed )
UA temporary prosthesis can be envisaged during this period.
If the implant can not s'osteointegrer it becomes mobile and painful and it must imperatively be removed. We will wait several weeks of healing and we can then sit another implant.

Several factors can prevent this osteointegration:

  • diabetes and an important imbalance.

  • a hygiene defects.

  • high consumption of tobacco (15 cigarettes a partirde \ update failure rate multiplied by 3.5)

The operation lasts between thirty minutes and two hours under local anesthesia. Les points de sutures s'ils ne sont pas resorbables sont retires après 8 jours. The stitches are not absorbable they are withdrawn after 8 days.

Exposure of the implant

This second phase is to reopen the gum and screw in the implant part specification:

  1. Gingiva (allows the healing of the gums)

  2. inlay-core that will support the crown or dental prosthesis.

Prosthetic Phase

After 3 months for the lower jaw and 6 months for the upper jaw we spend a prosthetic phase.
An imprint will realize a model for the laboratory. From this model, the laboratory performs the implant prosthetic phase.

Why do we need an implant?

The implant as a means of anchoring a dental prosthesis fixed or mobile unit or plural, represents a reliable functional and aesthetic treatment.

1- Fixed dental implant.
  a-Implant Unit
He has avoided touching teeth to make bridges

  b-Several implants
He avoids wearing an appliance (denture) that must be removed to be cleansed. Il est conseille un implant/dent manquante dans le cas de prothèses fixes. It is recommended an implant / tooth missing in the case of fixed prostheses.

2-Dental Devices
  a-Mobile
In case the patient needs a complete denture, it greatly improves the stability of the prosthesis and reduces the surface of the device.
The number of implants was chosen based on the oral condition and the number of missing teeth.

Sous une prothese mobile cela depend de l' etat de l'os (en general il faudra plus d'implants a la machoire superieure qu' a la machoire inferieure). On a mobile prosthesis that depends on the state of the bone (usually it will take more than the upper jaw implants that has the lower jaw).

On a mobile prosthesis two techniques are possible:

  • The retention bar

  • snaps

  b-Trans-screw
This will prevent the removal appareil.Tous 6 months to clean the cabinet will be made by the practitioner

The result depends on:

  1. From the surgeon's expertise (experience and professionalism) surgery is a specialty which requires four years of study after the degree of dental surgery.
A good number of internships allow a few days whether to place an implant, but these techniques must be used with pudences and competent practitioner can assume eventual infectious complications they can cause.

  2. Local factors:: quantity and quality of bone.

  3. Generals factors (power of healing and health status of the patient).

In case of deficiency bone marrow transplant will solve the problem of insufficient bone stock.
The financial factor is not negligible either.

To implant the bone must be of good quality and a wallet full.

There are different techniques of bone reconstruction for maxillary implants in satisfactory condition.

  • The levy bone Crane, Menton, Ramus, hip

  • When there is no great loss of substance, we avoid the levy by using synthetic materials, animal or human.

Fillers sinus

hey are shown in the bone deficiencies in the areas posterior to the upper jaw (located between sinuses and oral cavity).
The sinus is a cavity and to place the implant (titanium screws) is partially fills the cavity. Actuellement on utilise de plus en plus de materiaux synthetiques pour combler cette cavite. Currently being used more and more synthetic materials to fill the cavity. Après comblement aucune perturbation jusqu'a present n'a ete notee chez le patient. After filling up any disruption this has been denoted by the patient.

Clerk affixing

They are specified in a lack of bone thickness.

Different materials

Bone grafts pemettent increase the possibilities of implementation.

  • Autogenous graft from the same individual.

  • Allogeneic-from an individual of the same species.

  • Xenotransplantation, from a different subspecies.

  • Synthetic alloplastic graft.

  • Resorbable or not resorbable membranes

Why is it important to choose a specialist?

It exists in Israel (unlike France) for a specialty dental surgery. In addition to the certificate of dental surgery which is issued after 6 years of education can specialize in maxillofacial surgery. As in medicine the dentist will examine another 5 years in a hospital setting only oral surgery (accidents, cancer, implantology) His professionalism will avoid many complications (complications increases with age and state of health of the patient)

Failures can occur but in smaller numbers at a specialist

    There may be:
  1. Rejection of implant
  2. Drowsiness temporary or permanent
  3. Impairment of sinus, nasal cavity or dental nerve.

breach of sterility, an error in the operating protocol, a job implant prosthesis poor quality can lead al'échec badly made. Il est donc important de choisir : Un spécialiste en chirurgie It is therefore important to choose: A surgeon

It is also important to choose a very experienced dental prostheses to succeed the prosthetic phase, both functional and aesthetic.

Some tips to follow

After the implant may be pain, you can swell and there may be bruising on the face. Vous avez donc à prévoir. You have to be planned. Des comprimes contre les douleurs, éventuellement des antibiotiques et des rinçages de bouche Comme après une extraction il faut éviter de prendre de la nourriture consistante de boire ou manger chaud de faire de l'effort physique.il faut se reposer jusqu'au retirage des fils Of pills against pain, possibly antibiotics and mouth rinses as after an extraction should be avoided consistently take food to eat or drink hot to make the effort physique.il must rest until the son Reprint

Despite the gene that provides AC it is very important to have a perfect hygiene brushing teeth and rinsing the mouth (except 24 hours after the operation) should then be multiplied sessions of scaling and maintaining a very strict hygiene because increased sensitivity of the gums in people with implants. In many cases it is advised to wear a splint for a difference of teeth implants do not support the shock after filling sinus is forbidden to blow for 24 h The port of the device is not advised until the son of enlevage

Conclusion

All these techniques are relatively easy to perform acts when they are performed by a competent practitioner.
